Mediators of allergy, such as leukotrienes or histamine are produced and/or secreted upon contact of allergen with IgE molecules on specialized cells such as basophils and mast cells. In contrast to histamine, leukotrienes are also synthesized and released in the case of the pseudo-allergies, reactions resembling allergy, but without involvement of any IgE molecules.
BÜHLMANN CAST® ELISA the Cellular Allergen Stimulation Test, can also be considered as an in vitro allergy provocation test. The collected patient's leukocyte blood fraction will be exposed in vitro to the suspected specific allergen. After incubation the cellular supernatant will be tested for the de novo synthesis of sulfidoleukotrienes by a highly specific ELISA.
This test detects IgE-mediated allergies, but also the pseudo-allergies. This latter kind of allergy is best known for Aspirin (ASA) and other non-steroidal anti-inflammatory drugs (NSAIDs) and various other drugs or food additives.

Specimen Type: Blood into EDTA tubes. It is recommended that systemically administered antiallergenic drugs such as antihistamines, corticosteroids or chromoglycic acid (DSCG) should be avoided before sample collection. In order to avoid any interaction with systemically administered drugs BÜHLMANN recommends stopping the administration, if possible, three to seven days before blood sample collection. The exact waiting period depends on different factors, such as pharmacokinetic and bioavailability of substances, duration and regime of administration etc. Patients supposed to suffer from drug hypersensitivity should not be tested for the drug immediately after an adverse reaction. A recovery phase of several weeks after the adverse reaction is recommended. Different studies propose the highest sensitivity of the assay by testing the patient within about half a year after the adverse reaction to drugs. Important: Blood samples from patients going to be tested in vivo like skin test or oral provocation, must be drawn before any exposure to the allergen happens
