The complement cascade plays an important role in the body's immunological defence system. Complement activity measurement is a direct indicator of abnormalities of the complement system but can also assist with the diagnosis of other conditions. Complement activity has been correlated with the active stage of systemic lupus erythamatosus, rheumatoid arthritis, cryoglobulineamia-vasculitis and some forms of nephritis.

The wako CH50 kit is suitable for measuring total complement activity in serum using a liposome immunoassay method. When the sample is mixed with the liposome and substrate, antibodies in the reagent combine with dinitrophenyl (DNP) on the liposomes and this antigen-antibody complex serves to activate complement in the sample. The activated complement cascade will break the membrane of the liposome releasing the enzyme G6PDH. The G6PDH reacts with NAD and glucose 6 phosphate in the reagent reducing the NAD to NADH. The conversion of NAD to NADH results in an increase in absorbance as measured at 340nm. The increase in absorbance is proportional to the complement activity in the sample.

The Wako CH50 complement kit is supplied as 2x20ml liquid liposome reagent, 1 vial of lyophilised substrate and 1x 20ml diluent. The reconstituted substrate is stable for up to 40 days when stored at 2-8C. The CH50 Total Complement assay range is 10-65U/ml and is suitable for use on automated chemistry analysers. The assay is calibrated using the CH50 Calibrator 997-43801 and the recommended QC is the CH50 Control Set 991-43701.
